    I have tried several times to contact the Illinois Affiliate with no return contact, can anyone tell me if they are still active?

    Try http://www.appillinois.com/html/Officer.php it has names and phone numbers. If you are using their contact form page, I have never had a reply to anything I sent through it. I don't know if they don't ever look at it or it doesn't work but the phone numbers should work.
