Merit Request Forms for Affiliates and Affiliate Schools
These updated merit request forms can be filled out electronically and emailed to merits@ppa.com.
Please note that merit requests must be requested at least two weeks before the event date to ensure an on-time delivery.

Affiliate Applications and Forms

National Award
The National Award is a special honor in recognition of outstanding services to professional photography.

The choice of the recipient is left entirely up to the Affiliates.  It may be given to a member or non-member, to someone in or outside the field of photography.

While the National Award is available on a yearly basis, it does not need to be given each year if no suitable candidate is nominated.  To give the National Award:

  • The affiliate’s annual affiliation fees must be paid.
  • Only 1 National Award may be presented per affiliate per year.
  • The affiliate must have the name of the recipient engraved in the space provided. PPA is not responsible for engraving.
  • The date of presentation should be engraved on the bottom line of the plate.
  • The individual may not receive more than one National Award from the same Affiliated Association during his or her lifetime.
  • The National Award must be ordered and paid at least sixty days in advance of the event where the award will be presented.

Presentation should be made at the affiliate event by the highest-ranking PPA official of the Executive Committee in attendance, or current Board member, PPA Past President, or PPA Councilor.

The cost of the plaque and pin is $236.00, which includes shipping.
